Better, faster, safer care through information sharing: Victoria’s statewide health information exchange with CareSync Exchange

Overview

The Department of Health, Victoria is the steward of one of Australia’s largest public healthcare systems, a network of 75 public health services, spanning major metropolitan hospitals, regional centres and small rural facilities serving Victoria’s 6.7 million residents.

The Department of Health, Victoria recognised a strategic opportunity to transform the flow of patient information across Victoria’s public health services, connecting clinicians with the records they need, at the point of care to enable clinicians to deliver safer, more informed care.

Partnering with Altera Digital Health, The Department of Health, Victoria developed and deployed CareSync Exchange, powered by dbMotion— Victoria’s secure, statewide Health Information Exchange—embedding a single point of access to patient information directly within each health service’s existing Electronic Health Medical (EHR) workflow.

The system is underpinned by landmark legislation, Amendment (Information Sharing) Act 2023 to the Health Service Act 1988, enacted in February 2024, transforming both the legal framework and the operational reality of secure patient information sharing across Victoria’s public healthcare system.

From fragmentation to connection

CareSync integrates seamlessly into the existing EHR environment, surfacing patient information from across public health services with no separate login or workflow disruption required.

Clinicians accessing CareSync Exchange can immediately view a consolidated patient record comprising:

  • Demographics – name, date of birth, Medicare number, address, emergency contacts and GP details
  • Encounter history – emergency, inpatient and outpatient visits, diagnoses, and discharge summaries from any participating health service
  • Diagnostics – pathology results and reports

Notifications alert clinicians when new information becomes available for a patient in their care. A ‘break glass’ function enables authorised clinicians access beyond standard permissions—with an audit and review process implemented to ensure both clinical speed and governance integrity are maintained.

Growth driven by understanding the clinical need

The most compelling evidence of CareSync Exchange’s clinical value is the trajectory of adoption. Since the system went live in June 2025, clinician uptake has grown consistently, driven not by mandate, but by demonstrable utility at the point of care.

Between June 2025 and the end of May 2026, overall usage data usage includes:

  • 8,266 unique clinicians using CareSync Exchange across participating public health services
  • 50,527 unique patient records viewed
  • 368 new clinical users on average per month system

These results reflect the Department of Health, Victoria’s purposeful approach to supporting adoption, with CareSync Exchange configured and implemented to fit naturally within clinical workflows. By making the system easy to access during care and ensuring it presents information in a way that is useful to clinicians, the Department of Health, Victoria has enabled clinicians to recognise its value in practice and continue using it.

Sustained month-on-month growth in new users is one of the clearest indicators that the system addresses a genuine clinical need.

Statewide data at scale: What is now visible to clinicians

CareSync Exchange brings together a large amount of patient health information from participating Victorian public health services and presents in a way that is easy to view at the point of care.

CareSync Exchange now holds health information for more than 50% of Victoria’s population. This coverage will continue to grow by the end of 2025/26 as Gippsland Local Health Service Network, Bayside Health’s Regional Care Group and Bayside Health Peninsula join CareSync Exchange.

Prior to CareSync Exchange, accessing patient information was time-consuming, requiring clinicians to contact public health services directly, wait for records to be transferred and manually reconcile information with the patient’s current presentation. The information now exists in a structured, secure, real-time format—available at the point of care, within the clinical system clinicians already use

Network reach: Metropolitan live, regional expanding

CareSync Exchange launched across Victoria’s major metropolitan health networks, with the following services now live and contributing data to the statewide system:

  • Austin Health
  • Bayside Health Alfred
  • Bayside Health Peninsula
  • Bayside Health Regional − Wonthaggi Hospital
  • Eastern Health
  • Gippsland Local Health Service Network
  • Monash Health
  • Northern Health
  • Parkville precinct health services: Royal Melbourne Hospital, Royal Children’s Hospital, Royal Women’s Hospital and the Peter MacCallum Cancer Centre
  • Phillip Island Community Hospital

The forward plan for CareSync Exchange includes expanding the scope of clinical information to ensure that CareSync Exchange continues to provide the necessary information to enable clinicians to deliver safe, high-quality care.

This expansion reflects The Department of Health, Victoria’s commitment to ensuring all communities, metropolitan to regional alike, benefit from connected, high quality care, supported by seamless access to patient information wherever it is needed.

CareSync Exchange is progressively being rollout across Victoria with all public health services set to use the system by mid-2028.

Beyond the numbers: What CareSync Exchange means for the sector

The significance of Victoria’s CareSync Exchange program extends beyond the usage statistics. It highlights the Department of Health’s leadership in delivering a statewide Health Information Exchange at scale, supported by clear legislation, strong governance, careful implementation and genuine collaboration with health services across a complex public healthcare environment.

Avoidable duplication of diagnostic testing, treatment errors arising from incomplete histories and delays in care caused by fragmented information are not abstract risks. They have real consequences for patients. CareSync Exchange directly reduces clinical risks, across Victoria’s public healthcare system.

This is what genuine digital health integration looks like: not a separate system for clinicians to consult, but a transparent layer of connectivity that surfaces the right information, to the right clinician, at the right moment in a patient’s care journey.
